Critical Care Outreach Team

The Critical Care Outreach Team is a specialist group of nurses who provide expert support to deteriorating patients across our hospitals.

They respond to acutely unwell patients on the wards, ensuring they receive timely assessment, treatment, and if needed, escalation to a higher level of care such as High Dependency or Intensive Care.

Working closely with ward teams, the Outreach Team offers clinical expertise, guidance, and training to help staff recognise and manage deteriorating patients. They also follow up with patients discharged from critical care, ensuring a smooth transition back to the ward and continued recovery.

With a focus on patient safety and early intervention, the team ensures that critically unwell patients receive the right care at the right time, improving outcomes and supporting staff in delivering high-quality care across our hospitals.
