The NHS app (NHS.UK) > gives you a simple and secure way to access a range of NHS services including being able to view your GP health record. If your GP has given you access to your detailed medical record, you can also see information like test results.
UHDB provides patients with two options to access their data.
Patient Knows Best (PKB)
PKB allows for real-time access to certain information and provides patients with the ability to view clinical correspondence, appointment information, and appropriate test results. It also offers the ability to complete questionnaires digitally, and connect with health professionals and carers together in one place, enabling patients to take control of their own health.
Subject Access Requests (SAR's)
Alternatively, you can access your information via a subject access request > which is a legal right provided to you under the Data Protection Act 2018.
Power of Attorney (LPA)
If you are an attorney or the donor on a lasting power of attorney (LPA) and want this adding to a patient health record, please contact us via email uhdb.lpoarequests@nhs.net and include either a copy of the LPA or access code, which can be obtained from the .GOV.UK link below.
