Health records and Subject Access Requests

During the course of your care, you will be asked for information about yourself.  We use this information to provide you with treatment and care to the highest standard.

It may be used to train and educate our staff, carry out research and to manage and plan the NHS. We will keep your health records secure and all information held on computers within our Trust is held in accordance with General Data Protection Regulation (GDPR).

If you have any queries or concerns regarding the way your information can be used, please speak to a member of staff. You have the right to request access to your own medical records. If you wish to do so please complete the Trust application form. Information regarding the application process can be found in the guidance notes.

All Subject Access Requests for the Trust are processed by the Subject Access Team who are based off site.
