UHDB Transition Team to host Healthcare Transition Event on 2 October 2024
The UHDB Transition team have announced they will be hosting an upcoming event aimed at supporting young people with long-term or complex health conditions as they transition from paediatric to adult services — taking place on Wednesday, 2 October, from 6-8pm, at the Lecture Theatre at our Royal Derby Hospital site.
This important event is specifically designed for young patients from age 13 and older, and their parents or carers, offering a comprehensive overview of the healthcare transition process.
Transitioning from children’s to adult services can be a challenging time for both patients and families, and this event seeks to provide valuable information, resources, and support to help ease this journey.
Attendees will have the opportunity to hear from experts in the field, who will be delivering talks on a range of essential topics, including:
- The 'Art of Being Brilliant' - A session focused on the future of young people as they navigate their transition.
- Supporting your young person through transition - Guidance for parents and carers on how to best support their child during this period.
- Consent and capacity in adulthood - An exploration of the legal aspects surrounding consent and decision-making as young people move into adult services.
- Meet the UHDB Transition Team - An introduction to the dedicated professionals who will be supporting patients through their transition, along with teams from adult services who will be taking over care.
In addition to these informative talks, the event will also offer an open forum where attendees can ask questions and discuss any queries they may have with the Transition team and other healthcare professionals at the Trust.
This is an excellent opportunity to gain insight into the transition process and to connect with the team that will be guiding young people through this important phase. The team operates within the UHDB’s Transition Framework, which provides a structured approach to transition planning. This framework ensures that all young people receive equitable, sustainable, and high-quality care as they transition into adult services, with a focus on continuity of care and patient empowerment.
By working closely with clinical colleagues, the Transition team is committed to improving transition pathways, creating individualised care plans, and providing ongoing support to young people and their families during this significant time in their lives.
