Anna Ward
Anna Ward is a Discharge to Assess/Rehabilitation Unit.
You have been admitted to us following a stay within an acute hospital.
You will be allocated a suitable bed upon arrival.
We are a nurse led unit supported by our ward doctor who works Monday to Friday with an on-call service to support out of hours.
Other staff, including occupational therapy, physiotherapy, social care assessors, discharge liaison nurses and discharge support officers, are available to support you. This team may be referred to as the Multidisciplinary team (MDT).
We will endeavour to make your stay as comfortable as possible. During your stay:
- the nurses and healthcare assistants will support and help you with your care needs.
- the ward doctor will review you on arrival to the ward, and will monitor you during your inpatient stay.
- the Therapy team will meet with you and set your therapy goals. Your therapy is ongoing during your stay.
- the housekeeper and patient experience facilitator will try to ensure that your stay is as pleasant as possible. They will involve you in activities on the ward, which can be catered to you. Please speak to the nurse in charge for further information.
What do I need on Anna Ward?
We encourage all patients to wear day clothes throughout the day, rather than nightwear. Please ask a relative or friend to bring in some day clothes, nightwear and toiletries if possible.
We encourage you to not keep valuables on the ward. If you would like anything placing in our safe please let a staff member know and they will be happy to help.
How long will I stay on Anna Ward?
We aim to keep your stay in hospital as short as possible. Every patient’s needs are different and therefore their length of stay will vary. We will give you an estimated discharge date when you arrive, and we will do our best to achieve this.
How long you stay also depends on your needs following your discharge from the acute hospital, for example:
- you may need further therapy support, or extra help on discharge.
- you may need a package of care, or a place in a care/residential home following the necessary assessments.
While you are on Anna Ward:
- the MDT will assess your ongoing care needs and arrange for any extra help or support at home before you leave.
- the nurses will ensure your discharge letter is completed and complete any relevant referrals required on discharge e.g. district nurse referrals.
- the nurses will ensure you have 14 days’ worth of medication to take with you.
Arranging your transport home
If a friend or relative is unable to collect you from hospital, we are happy to help arrange hospital transport with you. We will try to ensure that you are discharged on time, however all bookings and times are estimated.
