Virtual MDTs
ImpACT+ holds a virtual MDT (vMDT) each month for every primary care network in Southern Derbyshire and Derby City. These meetings are held on Microsoft Teams.
What is a virtual MDT?
The vMDT is attended by a Respiratory Consultant and one of the ImpACT+ Specialist Nurses. Any other health professional can discuss their patients with respiratory issues. It provides an opportunity for a two-way discussion about the patient with shared decision making on the next steps.
What kind of patients can be discussed?
Most of the patients discussed are those with chronic respiratory diseases such as COPD or asthma. There are lots of reasons the patients are discussed; they might be having frequent exacerbations, or there may be concerns the diagnosis is incorrect. There is no real bar to discussion – we are happy to discuss any adult who has a respiratory problem that the clinical team want help and advice with.
How long does the virtual MDT take?
The virtual MDTs are timetabled for 2 hours. However, we do try and accommodate those who are not able to stay for the whole time.
How do I refer to the virtual MDT?
Please complete the vMDT referral form and return at least 1 week prior to the vMDT via email to dhft.impact-plus@nhs.net.
